Lacuna offers both Independent Living and Family Caregiver models to individuals on the DD waiver.
Independent Living: Our focus is on empowering individuals to live independently in the community, with customized support that helps them maintain their autonomy while ensuring safety and stability. We provide assistance in daily living skills, community engagement, self-advocacy, and more.
Family Caregiver: We work closely with families to ensure loved ones can continue to thrive in their home, receiving the necessary care and support while maintaining strong family connections. We recognize the dedication it takes to care for someone at home, and we are here to provide both the individual and their family with the support they need to succeed. Through ongoing guidance, helpful resources, and respite services, we aim to ease the challenges families face, offering a partnership that benefits everyone involved.
All residential services include access to nursing support as needed to ensure individuals’ medical and health needs are met in a safe and caring environment.

Lacuna’s Mentorship services are designed to promote self-advocacy and empower individuals to take an active role in all aspects of their lives. For parents with developmental disabilities, we offer specialized training in child and infant care, ensuring they have the skills and confidence to care for their children effectively. Lacuna’s mentorship services are tailored to each individual’s unique needs, fostering personal growth and independence.

Our Supported Community Connection (SCC) services encourage socialization, improve adaptive skills, and provide individuals with the personal support they need to navigate and thrive in community settings. We offer support to participate in community activities, as well as support with skill acquisition, retention, and improvement. All activities are conducted in inclusive environments where participants can interact with their communities, ensuring a rich and diverse experience.

Lacuna is led by a licensed clinical social worker (LCSW) with extensive experience in behavioral interventions. This expertise is the foundation of our high-quality services across the agency. Our behavioral support services include counseling, consultation, and assessment for individuals on both the SLS and DD waivers. We focus on behavioral interventions, including reducing challenging behaviors, building new skills, and supporting individuals with co-occurring mental health diagnoses and symptoms. Our team collaborates with individuals, families, caregivers, programs, and community members to address behavioral challenges and emotional well-being, creating personalized plans that promote positive outcomes. We aim to develop long-term solutions that focus on each person’s strengths, needs, and desires.

About Us
Welcome to Lacuna! I’m Kate, the Founder of Lacuna. I’ve been working in Colorado's Intellectual and Developmental Disabilities (I/DD) services since 2008. I began my career as a Direct Support Professional (DSP), providing support in a range of services including Supported Living Services (SLS), day services, employment services, respite care, and still hold the title of host home provider. Over the years, I transitioned into leadership roles, managing programs and guiding organizations toward success. My experience spans working with both small, one-person agencies and some of the largest I/DD service providers in Colorado, giving me a unique perspective on the industry.
As the founder of Lacuna, I’m proud to lead an agency that bridges the gap between regulations and the personalized care that individuals and families deserve. At Lacuna, we provide access to a highly experienced multidisciplinary team, including experts in various supportive roles, all working together to ensure holistic, high-quality care for those we serve.
Serving Grand Junction and Colorado’s Western Slope is Tiffany Montaño. Tiffany is a Licensed Clinical Social Worker (LCSW) with 10+ years of experience supporting Colorado’s youth and IDD communities. Tiffany has worked as a Case Manager, Residential Director, and Director of Behavioral Health before getting back to her true north, therapeutic services. Tiffany is currently providing Behavioral Counseling, Consultation, and Assessment services.
